_module_preset=&#8221;a4d5bd0b-c6df-4efa-93df-b960974f4af4&#8243; global_colors_info=&#8221;{}&#8221;]<\/p>\n<h4>Eligibility Criteria<\/h4>\n<ul>\n<li>Children or youth with physical, intellectual, or emotional differences who face financial hardships.<\/li>\n<li>Reside in our community areas.<\/li>\n<li><span style=\"font-size: 16px;\">Registered Charities or non-profit organizations that align with our mission to provide meaningful assistance to children or youth.<\/span><\/li>\n<\/ul>\n<h4>Application Submission<\/h4>\n<ul>\n<li>All applications must be mailed directly to our Foundation&#8217;s address listed below.<\/li>\n<li><span style=\"font-size: 16px;\">Include a letter that clearly explains the reason for the grant request and provides a brief overview of the child&#8217;s situation, including the diagnosis and how the funding will benefit the child.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Include a concise description of the child&#8217;s medical or special needs diagnosis, highlighting the urgency or necessity of the funding.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Specify the timeline for the needed funds, including deadlines for treatments, therapy, or purchases that the funding will cover.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Clearly state the amount of funding requested, and if relevant, break down the costs that make up this total.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Submit medical documentation from the child&#8217;s physician or specialist to confirm the diagnosis and explain the necessary funds for the child&#8217;s treatment or care.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Provide a brief overview of the family&#8217;s situation, including any financial challenges or extenuating circumstances that make the grant necessary.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Provide clear contact information for follow-up communication, including a phone number and email address.<\/span><\/li>\n<\/ul>\n<h4>Review Process<\/h4>\n<ul>\n<li>Each application is reviewed individually on a case-by-case basis.<\/li>\n<li><span style=\"font-size: 16px;\">Our review process is thorough and considers the unique circumstances of each application.<\/span><\/li>\n<li>The review board evaluates applications based on alignment with the Foundation&#8217;s mission, ensuring that every project contributes to our shared goals and values.<\/li>\n<\/ul>\n<h4>Decision and Notification<\/h4>\n<ul>\n<li>Upon receipt of your application, you will receive an acknowledgment and be informed about the review timeline.<\/li>\n<li><span style=\"font-size: 16px;\">After the Review Committee&#8217;s meeting, all applicants will receive notification of the application outcome.<\/span><\/li>\n<li><span style=\"font-size: 16px;\">Approved funds for successful applicants will be sent directly to the organization or service provider rather than to the family.<\/span><\/li>\n<\/ul>\n<p>Grantees are kindly requested to submit a progress report within six months of receiving the grant, detailing how the funds have been used and the impact achieved.<\/p>\n<p>[\/et_pb_text][\/et_pb_column][\/et_pb_row][et_pb_row _builder_version=&#8221;4.27.0&#8243;
